Playhouse Project Application - Habitat for Humanity of Greensboro, NC

I agree that I will have my (grand)child(ren) at the playhouse presentation on either April 12, April 13, October 18, or October 19 at the Gate City ReStore. Exact time will be communicated and agreed upon at least two weeks prior to the presentation date.

I understand that both Habitat and the sponsor will take photos of my family and/or (grand)child(ren) with the playhouse builders, and that these photos will be shared with Habitat Greensboro supporters. I give Habitat Greensboro, its representatives, and Playhouse Project sponsors permission to and assign the right to use and reproduce my family and/or (grand)child(ren)'s name, image, likeness and/or voice in any and all forms of media including, without limitation, photographs, films, videos, tape recordings, and internet publications. I also authorize Habitat Greensboro to use and distribute for any or all purposes letters, stories, testimonials, other writings attributable to me. I make no further claim of any nature of or connected with said use on my name, image/likeness, written or verbal statement made by me.

It would be a financial hardship to otherwise purchase a playhouse for my (grand)child(ren).

I understand the size of the playhouse is 4’ wide x 5’ long x 6’ high and that it is made of wood. Habitat Greensboro will not disassemble the playhouse for pick-up and delivery. It will take 3-4 strong individuals to load and unload it. You will need a truck to transport as it will not fit in a van.

I confirm I have space for it at my house and I have verified that it will fit through any doors or gates in order to place in my backyard.

I agree to pick up the playhouse, if possible. If I am not able to pick up, I will notify the appropriate Habitat Greensboro staff person at least one week prior to the playhouse build date.

I understand that the playhouse may not be dry by the pick-up time, and that there may be a short wait until it dries. I agree to make arrangements wtih the appropriate Habitat Greensboro staff person to pick up on a designated day/time.

I understand that Habitat Greensboro will receive many applications to receive playhouses, and I may not receive one at the original specified date or location. If this happens, I understand that I will be placed on a waiting list to receive a playhouse at a future date.

I understand that media may be present, and that I may be asked to speak to the media. If I am not comfortable speaking with the media, I will communicate that to Habitat staff.
